About This Notice
This notice supplements the Privacy Notice for health and care information handled through the household-planning service. Contact support@myplanstead.com with “Health privacy.” Do not include medical details in an initial email.
Information Involved
Information may include care and assistance preferences, long-term-care plans, medical decision-maker and provider relationships, health-related insurance references, medical-document locations, saved care resources, and health details in notes or uploaded files. A care search linked to an account may reveal a care interest.
Planstead does not request a clinical record, connect to electronic health records or device health feeds, or use geofences around health facilities. Include only information needed for planning and that you have permission or legal authority to provide.
Sources and Purposes
Information comes from the person entering it, authorized household participants, saved planning interactions and providers operating the requested functions. It is used to save, organize and display requested care-planning information, support authorized household access, respond to requests and protect the service.
Planstead does not diagnose conditions, monitor patients or recommend clinical suitability. A high CMS rating does not establish that a facility offers a particular service or can meet a person’s needs.
Searches and Recipients
CMS searches send the selected general location, facility name and filters to CMS. NPI searches send provider search terms. Household records, diagnoses and private notes are not included in those search payloads. Opening an external resource uses that independent website’s practices.
Saved records can be accessible to current household members under their roles. A listed person is not automatically a member. The active service providers listed below process relevant information to deliver the application, database and private files. Support systems process information you choose to include in a request; minimize it. Authentication email does not include private care-plan contents.
Authority and Separate Choices
A family relationship or household role does not establish authority to provide or share another adult’s health information. Terms acceptance and Privacy acknowledgement are not health consent. Review actual member access before adding sensitive information.
This service does not present a nationwide consent control as satisfying every state’s law. Any additional consent required for a particular collection, purpose or sharing scope must be assessed separately. Do not provide optional information for another person without the required permission or authority.
Advertising and Other Processing
Website visitor analytics collection, advertising pixels, product email, SMS and external AI processing of household records are disabled. Care searches and records are not sent to those services. No device health integration or clinical monitoring is offered.
Requests, Withdrawal and Appeals
You may ask about collection, access, recipients, correction, withdrawal of an optional permission or deletion. Email support@myplanstead.com with “Health privacy request.” You do not need an account or the household Owner’s approval to make a request about yourself. Identity and authority may need proportionate verification.
For an appeal, use “Health privacy appeal” and your request reference. Applicable rights, exceptions and response periods depend on the law governing your request. You may also contact the relevant regulator or attorney general.
Retention and Deletion
Removing a field, submitting a closure request and completing deletion are different operations. Requests may involve active records, private files, processors, shared records, backups and downloaded copies. Contact support for the exact scope and status; no instant or fixed backup-erasure guarantee is made.
Version and Changes
Version 2026-09-14.1 · Effective September 14, 2026. A notice update is not permission for a new purpose or sharing scope where separate permission is required. Review the general Privacy Notice and Data Lifecycle notice for additional information.

Information used: Web requests and operational records.
Supabase
Provides authentication, the household database and private file storage.
Information used: Account information, household records and uploaded files.
Resend
Delivers account confirmation and recovery emails. This does not enroll you in marketing.
Information used: Recipient email address and account-message content.
Providers process information needed for their functions. A provider certification does not certify Planstead.
